That the information I have submitted with this application is a true and complete record of all academic results I have achieved at each university and post-secondary institution I have attended.

I UNDERSTAND

That if I fail to provide my true and complete academic record, or if I misrepresent or have misrepresented my true circumstances, I may be excluded from studying at universities to which I applied, and that the university may terminate my studies at any time.

Giving false or misleading information is a serious offence.

The university may vary or cancel any decision it makes if the information I have given is incorrect or incomplete.

All documents submitted become the property of Emanate Resourcing and the universities and will not be returned.

Information is collected on this form and during my enrollment to meet the universities’ obligations under the ESOS ACT and the National Code 2007, to ensure student compliance with the conditions of their visa and their obligations under immigration laws generally.

The authority to collect this information is contained in the Education Services for Overseas Students Act 2000, the Education Services for Overseas Students Regulations 2001, and the National Code of Practice for Registration Authorities and Providers of Education and Training to Overseas Students 2007.

That should I be awarded a scholarship from any of the universities, I may not be eligible to hold another equivalent award at the same time.

That should I be awarded a scholarship from any of the universities, I will be required to repay that university to which I was awarded a scholarship from, any scholarship(s) that I have received based on false or misleading information included in my application. Scholarship benefits may include living allowances, fee remission and travel allowances.

I AM AWARE

I am fully responsible for my educational and living expenses while studying at university.

The universities, Emanate Resourcing, or the Government cannot assist me if I do not have sufficient funds to support my candidature.

I AUTHORISE

The universities and Emanate Resourcing to make any enquiries, including obtaining or verifying any records and/or other information from or with any education institution, tertiary admissions center, or other organization (whether education or otherwise), or individual, if it considers it necessary in connection with my application, enrollment and/or candidature.

I AGREE

To comply with the rules on admission and enrollment of the universities.

To inform the universities and Emanate Resourcing if there is any change to the information I have given in this application.

That the information I have given in this application is true and correct.
